If you left a pet in your home, please complete this Pet Retrieval form for each pet.               

You will be contacted with instructions on where to pick up your pet AFTER the team has retrieved your pet.

The personal information on this form is collected solely for the purposes of retrieving pets left behind during evacuation and is necessary for administrative purposes and may be shared with other public bodies, organizations and/or agencies to enable the provision of retrieving pets during the ongoing emergency. Questions regarding the collection, use or disclosure of the information should be directed to the City of Yellowknife.

The City of Yellowknife and its responders will not be liable for actions taken or not taken to retrieve your pet(s) from your home in an emergency. By signing this form:

• Your are confirming that you have the legal right to give permission to the City of Yellowknife and the SPCA (and those assisting) to entering your home;
• You are giving consent for City of Yellowknife and the SPCA responders to enter your home;
• You are agreeing that the City of Yellowknife and the SPCA and its responders will not be liable for actions undertaken in good faith to retrieve your pet(s) from your home in an emergency; and
• You are giving consent to the City of Yellowknife to use and disclose the information provided on this form to other public bodies, organizations and/or agencies who need the information to assist in the retrieval of your pet and reunification with you.
